Clinical history:

83 F with subconjunctival “salmon-colored” lesion.

Special stains:

: Kappa light chain ISH

: Lambda light chain ISH

: PAS – This stain highlights Dutcher bodies, intranuclear aggregates of glycosylated immuglobulin within the neoplastic plasma cells.

Clinical history:

68 yo M with large supraglottic mass, found during evaluation for hoarseness

Immunohistochemistry & In-situ hybridization:

: CD10 – The neoplastic B cells are negative for CD10.

: CD2 – A sparse but diffuse infiltrate of reactive T cells is present

: CD21 – Residual small meshworks of follicular dendritic cell processes are scattered throughout the infiltrate, a characteristic finding in MALT lymphoma.

: CD43 – The B cells are negative for CD43

: CD5 – CD5 is only seen on T cells, with no coexpression on B cells.

: Kappa light chain ISH – A few reactive kappa-positive plasma cells are seen around vessels.

: Lambda light chain ISH – There is a marked excess of lambda-positive plasma cells compared to kappa, consistent with a neoplastic process.

: Pax-5 – Most of the small lymphocytes in the infiltrate are B cells.

Clinical history:

Septal biopsy and excision of sinus contents in a 31 yo lady. Has had 2 prior biopsies in past year, both called “severe chronic rhinitis/rhinosinusitis.”
